Both changes reduce its affinity for IGF binding proteins, so more of it stays free in a culture system than native IGF-1 would. Why the LR3 modification?

Native IGF-1 is largely bound by IGF binding proteins. The arginine-3 substitution and the N-terminal extension lower that affinity, so a larger free fraction is available in the system.

Is it more potent than IGF-1?

In systems containing binding proteins it behaves as though it were, because more remains free. In a binding-protein-free assay the difference narrows.
